2/15/2023 EnchantaVaughn Approved Behavior: During in-home therapy hours, client demonstrated positive affection towards her counselor. Client appeared to be happy as evidenced by client smiling. Client is seen conversing and responding well to feedback. Client was able to cope and relax in her environment and contribute to the session. QMHP engaged mother as well as client in a discussion about behaivors since the last session. Mother as well as client discussed how client got suspended from school for fighting. Behavior: The family showed positive interactions during todayâs session. They interacted well with one another and spoke to each other calmly and effectively. The family laughed and joked with one another BUT STILL needed direction on their dialogue and interactions. Client also still needed redirection from QMHP as client verbalized to QMHP on how she is not going to let anyone just talk to her any type of way and she not react and this was demonstrated throughout todays session. Intervention: QMHP coached client to acknowledge and list her strengths, resilience and the positive outcomes that often occur when she makes informed decisions opposed to impulsive ones. QMHP verbally identified the client's impulsiveness to the client and encouraged the client to use self-control strategies (i.e. decrease hyperactive behaviors. QMHP reviewed several strategies for conflict resolution and positive social behaviors with client in efforts to increase clientâs awareness. The client was encouraged to use good judgement, and respect authority figures. Intervention: QMHP provided client with a release of energy by standing up and stretching to help her relax and open up more. QMHP talked to client about her confidence levels and how they can affect her thoughts and behaviors. QMHP provided simple praises to client for her commitment to her progress. QMHP praised and recognized client for her hard work, efforts, participation, and working towards improvement by changing daily habits and implementing a positive and consistent routine. QMHP taught client coping skills to assist her with being able to manage her anger within the school, home and community setting. Response: Client transitions smoothly and safely to session area, displaying appropriate behaviors. Client discussed mental illness and its causes as a family. Client exhibited understanding, demonstrating she was listening and retaining the information provided. Client was acceptive of support and encouragement. Client was able to reevaluate herself and display a more positive attitude toward peers and authority. Response: Client demonstrated expectations and responded well to feedback. Client verbalizes positive feelings about herself but recognizes areas she needs to work on. Client interacted well within her home environment., and the success of this was reviewed. Client implemented the positive reinforcement provided by her counselor and is able to stay motivated and enjoy her day. Client exhibited good manners after earning incentive for her hard work.
